CCI Assayer / PI platform provides guidance through chart abstraction of de-identified patient data and can automate the entire process of PI plan development, implementation, and re-assessment. Further, the system enables physicians to reflect on personal knowledge assessment along with practice-based data to report the impact of improvement changes. This supports the development of a
culture of continuous assessment and improvement within organizations like yours.
NetHealth's PI CME activities are based on the American Medical Association learning model for long-term performance improvement, guiding health care providers through the following process:
- Initial Data Collection: 10-30 charts are entered related to the evidence-based guidelines or ‘quality measures’ being evaluated. The system provides numerical and visual reports, identifying the recommendations for improvement, based on the data entered.
- Improvement Phase: Specific measures for improvement are selected and a personalized Performance Improvement Plan, guided by industry validated guidelines to set goals, is developed. Performance measures are briefly discussed and links to information sources, guidelines, and improvement tools are provided.
- Remeasurement & Reflection: After applying the improvement plan for a specified period of time, an additional set of data from another set of patient charts is entered to assess performance changes in comparison to the first abstraction. The final set of questions facilitates reflection on the results and the overall impact of PI process on quality of care. The data are then submitted for credit (CME / MOC / CNE, etc.) and the process is complete.
